He graduated from F. Hermann’s Real School in Riga in 1912 and studied at the Imperial University of Yuryev (now Tartu University) from 1913 to 1919.

While studying, he worked as a junior physician of the 280th Sura Infantry Regiment (1917–1918, Dvina Front), a junior physician in the dressing detachment of the Latvian Infantry Regiment (1918–1919), and as a volunteer physician at the 1st Riga City Hospital (1918–1920).

In 1920 he worked as a school and orphanage physician for the Daugavpils City Administration. From 1920 to 1923 he practised medicine in Višķi, Daugavpils County, and from 1923 to 1941 he served as a district physician in Ugale Parish, Ventspils County.

During the war he was evacuated to Kyrchany, Kirov Oblast, where he worked as chief physician of the Kyrchany Hospital from 1941 to 1945.

After returning to Riga, he worked as chief physician of the Mežaparks Sanatorium (1945), head of the medical unit of the Riga River Navigation School (1945–1947), district physician and later chief physician of Riga Polyclinic No. 10 (1947–1951), school physician at the 4th Riga City Polyclinic (1951–1952), and senior school physician at the 5th Riga City Polyclinic (1952–1968).

He was married to Rakhel (Rachel) Yakovlevna Rafailson (née Katz) (1893, Riga). Their sons were Yakov Davidovich Rafailson (1919, Riga – killed on the front in 1942 during the Second World War) and Yuly Davidovich Rafailson (1923, Riga).

In 1971 David Rafailson received permission to emigrate to Israel, where his nephew Yehezkiel Nisim Zundeleovich (1930) lived.
